Family and Caseworker

Submit the intake once, respond to funding requests, and wait for Elite to confirm authorization before services are scheduled.

Start Here

Use the public intake form for a new participant. For a new program on an existing Elite CLTS member, contact the appropriate Programming team instead of submitting a duplicate intake.

Your Responsibilities

  • Provide accurate participant, guardian, caseworker, club, and service information on the intake form.
  • Caseworkers review funding emails and send authorization decisions to Elite.
  • Families work with Membership on the contract and with Programming on consultation details.

Workflow Steps

  1. Submit the intake

    Complete the public form once for each new participant. Elite confirms receipt and reviews the request.

  2. Membership funding

    Business Office sends the dues funding request to the caseworker and an update to the family.

  3. Membership setup

    After approval, Membership contacts the family for contract and onboarding work.

  4. Programming consultation

    The selected department contacts the family to determine the exact package, dates, lessons, or sessions.

  5. Programming funding

    Business Office sends the requested program authorization to the caseworker and updates the family.

  6. Approval and service

    Elite confirms approval before programming starts. Contact the Programming team later if an additional service is needed.

Important Guardrails

  • Do not submit a second intake for an existing participant. It will be blocked and routed to CLTS administration for review.
  • Do not schedule or begin service until Elite confirms programming funding approval.
  • For changes or additional services, contact Elite rather than replacing prior funding paperwork.
